Guadalupe Health (“we” or “our”) is committed to protecting the privacy of our patients’ health information. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) to carry out treatment, payment, or healthcare operations, and for other purposes permitted or required by law. It also describes your rights regarding your PHI.

How We May Use and Disclose Your PHI

Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services. For example, we may share your information with your doctors, nurses, technicians, or other personnel involved in your care.

Payment: We may use and disclose your PHI to bill and collect payment for the services we provide to you. For example, we may need to give your health plan information about a procedure you received so they can pay us.

Healthcare Operations: We may use and disclose your PHI for our healthcare operations. These uses and disclosures are necessary to run our organization and ensure that all our patients receive quality care. For example, we may use your PHI to review our treatment and services and to evaluate the performance of our staff.

Other Permitted and Required Uses and Disclosures

We may use or disclose your PHI in the following situations without your authorization:

You have the following rights regarding your PHI:

  • Right to Inspect and Copy: You have the right to inspect and obtain a copy of your PHI that we maintain.
  • Right to Amend: If you believe the PHI we have about you is incorrect or incomplete, you may ask us to amend the information.
  • Right to an Accounting of Disclosures: You have the right to request an accounting of certain disclosures of your PHI that we have made.
  • Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose about you.
  • Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location.
  • Right to a Paper Copy of This Notice: You have the right to a paper copy of this notice upon request.
Changes to This Notice

We reserve the right to change this notice and make the new notice apply to PHI we already have as well as any information we receive in the future. We will post a copy of our current notice at our office and on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the Secretary of the Department of Health and Human Services. To file a complaint with our office, contact our Privacy Officer. You will not be penalized for filing a complaint.
